It is understood that one-piece implant design is a stronger concept as there is no connection between implant and abutment. The absence of a microgap can lead to minimal peri-implant bone loss. Furthermore, there is a reduction of mechanical complications such as screw loosening and abutment fractures. These implants can be immediately placed and can be put through instant function because of their high cortical stabilization. This immediate function protocol has advantages over two-stage surgical placement. Other benefits are fewer surgical appointments, reduced treatment time, and minimal trauma. It is suggested that one-piece implant can be an alternative to conventional implants for edentulous arches where there is a resorbed bone in width and height. Initially, one-piece implants were used as transitional or provisional implants. However, because of biological osseointegration, their removal often became difficult. Several manufacturers obtained approval for its use in defined situations. Advantages of Single Piece Implants / One piece Implants (OPI)
Mechanical durability:The OPI possesses lot more mechanical durability since , the implant cross section is strong as compared to Two piece implants(TPI) .As there is certainly removal of abutment screw in OPI , there is absolutely no empty space in the implant that provides adequate sturdiness to OPI even though the smaller diameter as compared to Two piece implants(TPI).

Marginal bone loss: One piece implant demonstrates minimized minimal bone damage since it does not have any microgap between implant and abutment. Thus the lack of alveolar bone around the implants is reduced because it is unable to have bacteria. Researcher Hermann JS, Broggini N, and Archie A. PROBLEMS OF ONE PIECE IMPLANTS

Abutments cannot be customised because the companies manufactures the implants, hence customization is impossible.

In some cases it may happen that the bone availability or bone thickness may be less.

Abutments will not have a finish margin. There will not be finish margin as the abutment is thin, so we cannot refine it more in the patient’s mouth.

As crowns are made manually, we cannot use CAD CAM software for single piece implants. Therefore we can never give precision in these implants.
CONCLUSION:Implant therapy is recognized as a reliable treatment to replace missing teeth; however, proper surgical placement of dental implant is necessary to obtain the required esthetic result. Through proper treatment the correct position and number of implants be determined. The field of implantology is continuously evolving and one-piece implants are becoming popular. One-piece implant design have high cumulative implant survival rate as well as beneficial marginal bone levels.
The goal of modern implant therapy entails more than just the successful osseointegration of the implant. Osseointegration being an accepted and well documented concept and now being established scientific phenomena is directed towards improving the design of implants, simplifying the surgical protocol, immediate placement and loading of the implants, and to reduce the restoration time. The design of an implant plays a key role in the success of a final restoration. The two basic designs which are available are the two piece implant design and the one piece implant design. This one piece implant design was originally created to eliminate the structural weakness built into the two-piece implant design (micro gap). This review of the literature will outline the advantages and disadvantages, indications, contraindications, implant design, surgical and prosthetic phase, as well as some of the bio-mechanical parameters which are required for its selection.
ADVANTAGES OF ONE PIECE IMPLANTS

Mechanical strength:The OPI has more mechanical strength because, the implant cross section is solid as compared to TPI14.As there is elimination of abutment screw in OPI , there is no empty space in the implant which provides sufficient strength to OPI despite of its smaller diameter as compared to TPI11,9.

Marginal bone loss: OPI shows reduced marginal bone loss as it has no microgap between implant and the abutment. Thus the loss of alveolar bone around the implants is minimized as it cannot harbor bacteria. Hermann JS et al15, Broggini N et al16, and Archie A. One piece implantology after wide spread diffusion in the second half of the last century saw its application significantly reduced by dental implantologists. The main reason of a progressive reduction was the Brakeman concept of implant Osseo integration, although now the most recent knowledge allows reducing the timing on implant loading. Another significant factor is attributable to the prosthetic difficulties compared to two-pieces devices and to the technological limits that have made the one piece implants not suitable for the paradigms of modern implantology. The intent of the authors is to repropose one piece implantology and highlight the use of medical devices designed and built in the past, but contextualized currently in light of the technological improvements available today so that they can be considered as a valid alternative to the others implant methods.
